[00:00:07] What is an Individual Health Plan? An Individual health Plan, or I H P, is an individualized plan for your child that explains the health condition and what to do in an emergent situation.

[00:00:07] What is an Individual Health Plan? An Individual health Plan, or I H P, is an individualized plan for your child that explains the health condition and what to do in an emergent situation.
[00:00:17] Step one, school nurse. Step two, severe asthma attack. Step three, rescue inhaler.
[00:00:25] An I H P will list signs of medical distress as well as medications needed for the safety of the child. The I H P will also list medical providers and insurance.
[00:00:36] The I H P can be a standalone document or can also be attached to a child’s 5 0 4 or I E P for additional descriptive information of the child’s medical condition and needs.
[00:00:47] When considering an individual health plan, meet with the medical provider as they may have a specific form for your child’s medical diagnosis, as well as valuable input written in the I H P that school can use. [00:01:00]
[00:01:00] More individualized medical information listed on the I H P will only benefit your child’s needs and possibly protect him from an emergency situation occurring, like severe asthma, access to school nurse, use inhaler when needed, things that can cause an attack like dust and smoke.
[00:01:16] Whether the I H P is a standalone document or attached to an I E P or 5 0 4 Plan, meet with the school administration to help get them insight to what your child’s medical needs are and the importance of following the I H P steps.
[00:01:29] Why use an Individual Health Plan? Parents and providers can work together to create this document, give detailed medical information to anyone working directly with your child in the educational setting.
[00:01:41] If a medical situation arises, the I H P will give detailed instruction on medication administration, and or individualized medical care to your child’s health condition needs.
[00:01:52] By creating and utilizing an I H P, this helps to prevent and assist in a medical emergency while in the educational setting. [00:02:00]
[00:02:00] Information needed for an individual health plan. A medical diagnosis such as asthma, allergies, A D H D, Cerebral Palsy, et cetera, treatment needed during school hours, medication needed, adjustments to the school day, if applicable. And individualized healthcare needs such as lifting, feeding, positioning, transportation, et cetera.
